Hello and welcome to the Geriatrics, Interprofessional Practice and Interorganizational Collaboration (GiiC) toolkit. Originally developed for family health teams and community health centers, the toolkit is now available to all health professionals who care for frail seniors. In the blue banner above you will see the names of the site's knowledge to practice modules. Each module contains topic overviews, quick facts, algorithms, pocket guides, tools, patient handouts in english and french and resources (slides and case studies) to help users teach these topics to others.
