Inter-organizational Collaboration

We are in an era of integration across the sectors of health care. Care of frail seniors has always been more than one organization can accomplish working alone. It requires the collaboration of primary care, community care, home and hospital based care providers. Ideally these providers a seamless continuity of service. But, just putting organizations together to work does not necessarily produce effective collaboration. Interorganizational collaboration requires continual care and tending. This page contains knowledge to practice resources on interorganizational collaboration. The resources are being developed by the GiiC project principal investigator and project director, David Ryan.
