Hello and welcome to the Geriatrics, Interprofessional Practice and Interorganizational Collaboration (GiiC) toolkit. Originally developed for family health teams and community health centers, and further developed to meet the needs of community health centers, the toolkit is now available to all health professionals who care for frail seniors. The toolkit comprises 24 modules on key areas related to the the care of frail seniors These materials are available without charge but access does require a simple registration.
GiiC is a collaboration of the Regional Geriatric Programs of Ontario, the Center for Research and Education on Aging and Health, and the North East Local Health Integration Network Specialized Geriatric Services Group. The GiiC website is an enduring legacy of grants provided by HealthForceOntario to support its Interprofessional Care Blueprint for Action . Translations provided by the French Language Health Services Translation Network, Ontario Ministry of Health and Long Term Care.
